Intermediate Form
Application for admission may be submitted online to GMC, Sialkot website and hard copy must be submitted by hand to the admission office within the prescribed time notified by the admission office. No Admission Form shall be accepted after the expiry of the last date for submission of Admission Forms except with the special permission of the Principal or Admission Committee.
The candidate has to submit admission form complete in all respects to the admission office along with following documents.
- Three (3) attested photocopies of result card of Matriculation
- Three (3) attested photocopies of School Leaving Character Certificate
- Three (3) attested photocopies of Student’s CNIC/Form-Bay
- Three (3) attested photocopies of Father/Guardian’s CNIC
- Four (4) unattested Student’s Passport size Photos with Blue Background
- Original NOC of concerned Board if Matriculation is other than BISE Gujranwala, File Cover
